a. FLU FORM - Complete "ALL AREAS" Front and Back & SIGN at the bottom (last page of packet).
b. School Immunization FERPA release - Fill in ALL blanks on 1/2 page (1st page of packet).

Who? Scholars, Faculty, & Volunteers who work the flu clinic.
Where? Haas Hall Academy-Bentonville Campus
When? Tuesday, November 10, 2015 @ 12:30 p.m.

How to sign up?
1) Get flu form from your scholar (sent home 10/27/15) OR pick up a form at HHA-Bentonville campus.
2) Complete "ALL AREAS" front and back of flu form & SIGN at the bottom.
3) Insurance information MUST be provided even if you have private, ARKids, or no insurance.
4) Return completed flu form before the Friday, November 6, 2015 deadline.

Can I volunteer to help?
YES! A few parents are needed to direct scholar traffic & NURSE volunteers are needed to immunize. (Nurse volunteers must contact our campus nurse for required nurse paperwork).

Haas Hall Lit Mag

Haas Hall's Literature and Arts magazine Footnotes is now taking submissions of all fiction, poetry, essays, photography, and art from both Haas campuses!

This is a general submission, meaning that submitted works do not have to confine to a particular theme.

Chili Social Tickets

Booster Club Chili Social & Silent Auction tickets are available. If you'd like tickets please email Jennifer McMahon at jingalls@hughes.net with your name and the number of tickets you need. The cost is $7.00 per person.
Once you let Jennifer know how many tickets you need, please send in your payment, made out to HHA Booster Club, in an envelope with your name on the outside, number of tickets needed and write Chili Social on it as well.
The Chili Social will be Thursday, November 5th from 5:30-8:30.
We can either hold your tickets until the night of the event or send them home with your scholar.

The Booster Club's 5th Annual Chili Social and Silent Auction will take place on Thursday, November 5th from 5:30-8:30 at Haas Hall Academy, Starr Scholar Center in Fayetteville.
Please consider signing up for one of the many slots available in our sign up genius.
This is our biggest fundraiser for our athletic programs.

Tyson Project A+ Labels

Dear Parents:
I have exciting news! Our school is participating in the Tyson Project A+™ program, a fund-raising program sponsored by Tyson Foods, Inc. The money can go towards supplies, building improvements, school programs, or anything else we choose. It's an easy way to raise much needed funds by sending Tyson Project A+ labels from products you probably already buy.
Here's how it works: Just clip and collect Tyson Project A+ labels from participating Tyson® product packages and send them to school with your child. We'll collect the labels and send them to Tyson Foods, and Tyson Foods will send our school a check. Each label is worth 24¢!
I'd be happy to answer any questions you have about the program. You can also find details online.
I hope you'll join us in collecting Tyson Project A+ labels this year and spread the word to your friends and relatives. It's such an easy way to raise much needed funds for our school!

Run for the Parks! Volunteers Needed

Looking for an autumn volunteer opportunity? Fayetteville Parks and Recreation could use your time and enthusiasm at our "Run for the Parks" race on November 22. This running event is one of many in the Fayetteville Race Series and will be held along the city's paved trail system. Runner's will have three different distance options to choose from, each of which starts at a different park and finishes at Walker Park.

Volunteers are needed to help with race packet pick up, participant check in, starting/finish line assistance, and monitoring/cheering along the course. Check out our event and the volunteer shifts at GivePulse.com
